What HCG Does With TRT

HCG (human chorionic gonadotropin) is added to TRT to preserve fertility and prevent testicular shrinkage — the two problems testosterone therapy causes on its own.

Here's why. When you take testosterone, your body senses it has enough and stops signaling your testicles to work. Left idle, they make less sperm and shrink. HCG sends that "keep working" signal directly, so your testicles stay active while you're on TRT.

It's an add-on, not a replacement. TRT handles your symptoms and testosterone levels; HCG keeps your testicles online.

HCG Protects Fertility on TRT

Does TRT cause infertility? On its own, often yes — testosterone therapy can drive sperm counts toward zero within a few months. That's the single biggest reason men hesitate to start.

HCG is how you treat low T without giving up fertility. By keeping the testicles stimulated, it maintains sperm production during therapy. If you want children now or want to keep the option open, this is the conversation to have before starting — it's far easier to maintain fertility than to recover it later.

Testicular shrinkage on TRT is predictable — most men see noticeable volume loss over the first year without protection. Some men don't mind it; many do.

HCG largely prevents it by keeping the testicles working. Added from the start, it maintains normal size. Added after shrinkage has set in, it often restores volume over several months, though prevention is more reliable than reversal.

How HCG Is Dosed and Prescribed

HCG is a small injection under the skin, usually two to three times a week (a low dose, typically 250–500 IU) alongside TRT. Men trying to conceive may need more. Dr. Fuzayloff sets your exact dose from your labs and goals — it's not one-size-fits-all.

We send the prescription to a pharmacy; we never sell medication in the office. You can use an online pharmacy we've arranged pricing with (delivered to you) or your own. Dr. Fuzayloff can also give the injection in-office.

Follow-up is simple: occasional bloodwork to keep your levels in range, with dose adjustments as needed.

Medication is separate

HCG is separate, purchased from the pharmacy. Generic HCG typically runs about $80–$200 per vial; a vial lasts one to several weeks depending on your dose. Insurance usually doesn't cover HCG for men, so expect to pay cash. Because of an ongoing national shortage, price and supply vary — we'll send your prescription and go over current cost at your visit.
